Michael was born on May 4, 1962 in Dover, OH and passed away March 10, 2012 in Chicago.
Michael is survived by his mother Esta Johnson; brother Edward Betz; loving sons Jack and Jake Betz; his former wife Mary Betz and his father Ken Betz.
