District 15 Walk-In Registration at End of July

Enrollment information and registration forms must be completed for your child to be registered for the upcoming school year.

Walk-in registration for District 15 students will be held at the end of July, and parents may want to begin gathering information required by the school district.

Parents need to provide the following:

  • A transfer slip from the previous school attended (if applicable)
  • An official birth certificate (not a hospital-issued certificate) or other government document verifying the date of birth
  • Physician's telephone number and address, if known
  • The name and phone number of two reliable local persons to call if you are unavailable in an emergency
  • Two proofs of residency (copies of drivers license, lease, or utility bills)
  • Any pertinent health information including health, dental, and eye exams at required intervals (see below for more information). A physical examination is valid so long as it was conducted on or after August 26, 2012.

According to District 15, enrollment information was mailed to parents of students who currently attend the school district, and registration forms should be updated and returned as soon as possible.

Students who are entering kindergarten are required to be five years of age on or prior to September 1, 2013.

The walk-in registration is for students who have not yet registered and will be held on Wednesday, July 31 at all the D15 schools from 7:30 a.m. until 1 p.m. and pick back up at 4 p.m. until 7 p.m.

 The first day of school for the 2013-2014 school year is Monday, August 26 for grades 1-8. Early childhood and kindergarten classes begin on Tuesday, August 27.
