District 15 Walk-In Registration for 2014-15 School Year to be Held in July

Walk-in registration for students who will be attending District 15 schools in the 2014-15 school year but are not yet registered will be held on Wednesday, July 30, at all District 15 schools. Hours are 7:30 a.m.–1 p.m. and 4–7 p.m.

Parents who are not certain which school their child will attend may call the District 15 office at 847-963-3000.

Children must be 5 years of age on or before September 1, 2014, in order to register for the 2014-15 school year.

To register a student, parents must bring:

  1. a transfer slip from the previous school attended (if applicable);
  2. an official birth certificate (not a hospital-issued certificate) or other government document verifying the date of birth;
  3. physician’s telephone number and address, if known;
  4. the name and phone number of two reliable local persons to call if you are unavailable in an emergency;
  5. two proofs of residency (copies of driver’s license, lease, or utility bills); and
  6. any pertinent health information including health, dental, and eye examinations at required intervals (see Enrollment Procedures for more information).

Enrollment information will be mailed over the summer to parents of students who attended District 15 schools during the 2013-14 school year. Registration forms should be updated and returned as soon as possible.

 

The first day of the 2014-15 school year will be Monday, August 25, for Grades 1–8. Kindergarten and early childhood classes will begin Tuesday, August 26. School supply lists for each school are available from the schools and will also be posted on each school’s page on the District 15 website at www.ccsd15.net | Our Schools.
